Please rename your document filename to contain your first and last name and add a short description of what the file is (i.e., JohnDoe_medicaldocument.pdf or JohnDoe_medicaldocument.doc). Please use only your UMBC email address in the submission box.
If you are unable to attach your documents now, you may submit them using the additional delivery options below. Please note, however, that using the mail delivery method might delay the receipt of your documentation and review and consideration of your request:
